5th victim of fatal Bangor plane crash identified

A sommelier who was traveling with a private chef was on the private jet that crashed in Bangor, Sunday evening, killing everyone aboard, sources close to the passengers told The Daily Mail.

Thirty-four-year-old Shelby Kuyawa was traveling with chef Nick Mastrascusa, 43, according to The Daily Mail. Six people were aboard the plane, including two crew members and four passengers.

The Bombardier Challenger 600, registered to an LLC connected to a Houston-area law firm, had stopped in Bangor to refuel and was headed to Paris when it flipped over and caught fire as it was taking off around 7:45 p.m. Sunday at Bangor International Airport.

Officials have not released any names, but family and friends have since identified four other victims, including Mastrascusa, Tara Arnold, Shawna Collins and pilot Jacob Hosmer. The identity of the second crew member remains unknown.

Arnold, Collins, Mastrascusa and Kuyawa were part of the luxury travel company Beyond, started by Tara Arnold and her husband Kurt Arnold, who was not on the plane.

The official cause of the crash is being investigated by federal officials. Information from flight tracking websites, air traffic control recordings and statements from local and federal officials in the moments leading up to the crash, paints a picture of a plane model with a history of failing in icy weather attempting to take off in a snowstorm.
